AlwaysOn Healthcare specializes in providing non-medical home care services. However, we prescribe to a holistic care approach using the Patient Centered Medical Home Model. With this approach, AlwaysOn Healthcare aims to encompass a patient’s total need, rather than simply focusing on just the non-medical in-home care services we provide!
With total client care as our goal, we can assist clients and their families by recommending other skilled medical or specialty in-home care providers, ensuring our clients stay in their homes to age in place as long as possible. AlwaysOn Home Care coordinates with other elder or adult skilled care agencies in addition to Hospice, Physical Therapy, DME, Doctors Offices, Skilled Nursing and Rehabilitation Facilities, as well as Hospitals for extra support.
